Interesting case from this morning: This patient presented with pain to biting and chewing for approximately eight weeks, gradually increasing in pain. The patient has a history of bruxing and is currently wearing a night guard. Slight thickening of the PDL can be gleaned on the periapical radiograph, but the scan gives it away. Sometimes being an Endodontist is knowing when not to do the root canal.

While not expressly considered the standard of care in Endodontic or root canal therapy This case illustrates just how helpful pre AND postoperative CBCT scans are. Four canals were located, consistent with my expectations, and the mesial and distal canals appeared to be roughly parallel. I felt comfortable with the post operative radiograph, but the post operative CBCT scan was less thasn flattering. (I've often called cone beam computed tomography, "the great humbler.") I realized I had missed the ml canal. The tooth was re accessed and the and previously untreated mesiolingual canal was subsequently located, cleaned and sealed.

The pre-operative and working CBCT’s allowed me to efficiently and effectively navigate the anomalous anatomy in this rotated Upper central incisor with a deep bifurcation approximately 12 mm from the lingual access.
