Experiences of endometriosis can be unique, just as responses to treatments are. But universally, patients want largely the same things: To be seen, heard, believed and valued as authorities on their own narratives To have access to gold standard care regardless of financial status To have emphasis placed on relief - or at least reduction - of symptoms - not just procreative potential... To make choices through truly informed consent - not based on the bias or agenda of the prescriber/practitioner And to be treated as partners in care. Patient-centered care and shared decision making starts with listening to and really hearing - individuals. Studies have shown that patients want their doctors to take a collaborative, human-first approach, communicate with their heart and mind, behave with integrity and practice competently. An endometriosis diagnosis is considered uncertain without surgical confirmation. Tests may be helpful in the larger sense, but are not definitive except in very limited circumstances (about 5% in the case of colonoscopy, for example). Laparoscopic Excision, however, allows for minimally invasive diagnosis and treatment, including for intestinal endo.

The incidence of adhesion formation depends on many factors. Patients with advanced stages of endometriosis (stage III and IV) often have a significant amount of adhesions even prior to any surgery, because endometriosis itself can cause adhesions to form. Over 70 years ago, Dr Clayton Beecham declared that ‘nature, since the beginning of time, has employed an efficient prophylactic and curative measure for endometriosis...pregnancy.’ Even years prior, however, Sampson, Meigs and others were writing about the alleged beneficial relationship between pregnancy and endo, noting the reduction of symptoms for some - albeit temporary (if at all). Such observations ultimately led to the prescribing of pregnancy and later, in the 1...950s, use of synthetic progestins as a treatment for endo. However, the assumptions that have carried over for nearly a century about pregnancy and endometriosis are based on short-sighted observations and outdated, limited and poor-quality evidence. Of course pregnancy is not a cure or treatment for any disease, and it cannot be prescribed as such. Integrative, multidisciplinary care is the key to effectively treating endometriosis - not considering pregnancy as a therapy, especially for those who may be struggling with infertility as a result of endo or who simply choose not to pursue pregnancy.
