Homestead Hospice

Volunteers and staff members with Homestead Hospice, Tifton, Georgia recently "Egged" several local assisted living facilities. They placed eggs filled with candy for residents to find and enjoy. Homestead Volunteer Coordinator, Kendra Vickers, Community Relations Managers, Tonya Payne and Vicki Rountree helped organize the activity and help spread joy to seniors living in local assisted living facilities.

It was a beautiful Spring day for the Shriners Smoke on the Beach BBQ event in Myrtle Beach, South Carolina. Homestead Hospice Community Relations Manager, Mike Molina and Director of Business Development, John Bolek (not in photo) had plenty of bottled water and treats to share with the community during the BBQ event. Homestead Hospice is proud to be part of this noteworthy cause.

The Nurse Case Manager directs the care of his/her patient caseload, communicates with other team members, families, and ALF/SNF staff. The Nurse case manager supervis...es the HHA and LPNs, fills out the appropriate paperwork in a timely manner, communicates with the Attending MD, and protects the human and civil rights of patients and/or clients and staff. The RN is responsible for directing the total care of the patient. The Nurse Case Manager coordinates the total care of the patient, including communication with the IDT members, families, Attending MD’s, and facility wellness coordinators. Paperwork is to be completed and turned into the office in a timely manner. Responsible for covering call at least 7 days per calendar month. Must have dependable transportation. Qualifications: Current nursing License in the state, with at least 3-year experience in home health/hospice Familiarity with a variety of the clinical concepts, practices, and procedures Ability to exchange routine information in an appropriate manner. Good oral and written communication skills Responsibilities: Day-to-day visit schedule, meeting the approved visit frequency Maintains a nursing service philosophy and related rules, standards and practices; establishes care plans, including assessments, plans of treatment, objectives and goals, evaluations, and discharge planning, maintains current medication profiles, and timely reorder of hospice appropriate medications Ensures compliance with laws and rules related to care, certification, and licensing; professionally represents Creative Hospice Care to residents, residents’ families, outside agencies, care providers, and the public Maintains liaison role with all levels of administration, physicians and outside organizations Complete required documentation in the time frame allottednurses notes turned in at least 2 times per week, admission packets turned in within 24 hours of the admission, discharges turned in within 24 hours of the discharge, recertification packets turned in by the due date of recertification’s Is responsible for on call services, at least 7 days per calendar month. Homestead Hospice is so thankful for all the individuals and groups of people that volunteer with us every day. The Classical Conversations homeschool group in Newnan is one of those volunteer partnerships we are grateful for. They have been volunteering for Homestead in Newnan, GA for almost a year. Recently, the group created chalk art for Watercrest of Newnan assisted living (Watercrest) who is a Homestead facility partner. The children also dropped off a basket of snacks for residents at Watercrest. Thank you to the junior volunteers and the parents of Classical Conversations homeschool group!

Residents at Legacy Village, in Tifton, GA are frequently visited by Tess, a Homestead Hospice therapy dog. Her owner Homestead RN Case Manager, Ashelee Wood tries to include Tess on her visits to the assisted living whenever possible. Resident Ms. Myra absolutely loves Tess and Tess enjoys making the residents happy.

Happy Friday Everyone! Meet Homey the Homestead scarecrow! Kelly Minish, Community Relations Manager in Athens GA, created and submitted Homey into the scarecrow competition at Manor Lake of Athens assisted living. Great job Kelly, we hope Homey the Homestead scarecrow wins!

Today at the Homestead corporate office we celebrated our CEO Mallie Sharafat's birthday! As Mallie put it, she will be "26" forever! Forever Young! Happy Birthday Mallie.
