Beautiful healing on a couple of 12-month recalls this week.

I recently completed #’s 13 & 15. The 4th canal, Mb2 occurs in over 90% of 1st molars and 50% of 2nd molars. One nice use of the CBCT for me has been to help verify if MB2 is present. The bottom right Scan was taken with Ca(OH)2 in the MB, DB, and P. The scan helped guide me to the MB2 at the second appointment. It’s extremely rare to see a maxilllary 1st molar with only 2 canals,so when I was a dealt molar with 2 roots and 2 orifices, I figured the buccal canal was up to some trickery. As an endodontist, sometimes I’ll get referrals for cosmetic internal bleaching. This was a 45 yo male with a history of childhood trauma. The pulp canal was completely obliterated/calcified, but fortunately he had no signs or symptoms of an endodontic infection. In cases of pulp canal obliteration, studies show the chances of future infection are less than 10%, so the odds are really good that he’ll never need a root canal. These teeth can still be effectively bleached internally by cutting an access of sufficient size in the lingual of the tooth and placing your bleaching agent of choice. We got a nice result, and now he’s ready for external bleaching with trays to even out the color for the rest of his teeth. #cosmeticendodontics #thinkingoutsidethebox #toothwhitener

Many times a case like this never reaches my door because the referring doctor assumes it’s cracked. I’m pretty lucky to work with some very smart restorative dentists. Ahh, the doomed necrotic molar with no restorations or caries. As suspected, we observed a deep crack along the distal w/ an associated narrow 12 mm isolated probing. Under the scope we confirmed the crack extended into the distal root. I also noted that it was a c-shaped molar, which has a fused root morphology that is structurally weaker and more prone to this pattern of fracture than the traditional multi-rooted molar.

Today I had 2 patients with 2-rooted lower canines. The incidence of 2-rooted lower canines in the general population is ~5%, so it was pretty strange to see this in 2 different patients on the same day. The lower set of x-rays was of a young female who was in a car accident and had trauma. There was concern that #22 had a horizontal root fracture in the apical third based on the radiograph. The CBCT revealed that this radiograohic finding in was simply the superimposition of the two root forms. #cbctsavestheday!

Snagged a 15-month recall this morning. Patient had significant pain and swelling at the initial appointment due to a missed second canal in #25. The literature states 1/3 of mandibular incisors have 2 canals. Anatomy failures in endo tend to heal readily when the untreated anatomy is addressed. #cbctguidedretreatment #iloveendo
