We are often asked about meds for treating endometriosis. Sadly, there is nothing we can inject, ingest, inhale, or roll around in that can remove even one speck of endo from the body. The only way to do that is through surgery, preferably excision. Occasionally, when a patient has to delay surgery (for example, a teacher waiting for summer vacation), Dr. K feels that there may be a role for short-term suppression, as with a birth control pill, to ease symptoms. This study: https://journals.lww.com//Elagolix_Versus_Laparoscopic_Sur shows that Orlissa is less cost-effective in treating endometriosis than laparoscopic surgery. Duh! Most patients find that excision surgery that completely removes all endometriosis is best in terms of long-lasting symptom relief, improved fertility and enhanced quality of life. On our Lunch & Learn call today Dr. Nick was asked about when a person should have surgery. He said, 'I tell patients, you should have surgery when you tell me it's time'. He then went on to explain that some things, like large nodules, giant endometriomas, invasive bowel disease, etc. call for a more urgent trip to the OR, but if a patient's symptoms are manageable, immediate surgery can be deferred. However, when daily life activities are curtailed because of pain, or patients desiring pregnancy have been unable to conceive, surgery should be considered. Finally, even if symptoms are manageable, endometriosis can become more invasive over time, so it's important to consider that as well. During today's Lunch & Learn, Dr. Nick talked about bladder endo. He explained that endo on the outside of the bladder can cause symptoms similar to a UTI, but when the urine is cultured, nothing grows. Some patients endure repeated rounds of antibiotics with no improvement (because infection is not the problem). Happily, bladder endo can be safely and completely removed at excision surgery. On today's Lunch & Learn we talked about adenomyosis. Dr. Nick explained that having this diagnosis doesn't mean hysterectomy is inevitable. First of all, the only sure way to diagnose adeno is AFTER a hyst; before then it's just a guess. You might not have it at all. Next, if symptoms such as heavy bleeding and severe midline pain are not present, then even if the diagnosis is correct, nothing needs to be done. This is especially true if the patient's goals include childbearing. And even if you have these symptoms, there are some treatments to try first (hormonal manipulation, pre-sacral neurectomy, etc.). The point is, you have options. Talk to your physician and ask alllllllll the questions. We love you and want you to be well. xo
