The SHORT Case Of The Day! Tooth #2 ~ WESTSIDE! Patient was referred from his dentist. He was in pain. The diagnosis was a previous root canal with acute apical periodontitis. Endodontic retreatment was performed in 2 visits using calcium hydroxide. Patient returned in 2 weeks asymptomatic and the root canal case was completed. The canal anatomy was very unusual. The frequency of reports on two palatal roots with two canals on a mx 2nd molor is low. Peikoff et al. reported a... 1.4% incidence of four separate roots. You are getting a peak at a rare sighting! A New Study In JOE on CBCT & Detecting Fractures A fracture width >120 m has been reported to be better visualized by CBCT imaging which is approximately the size of a 15 handfile. From this systematic review and meta-analysis, it can be concluded that the available evidence is weak with regard to the diagnostic ability of CBCT imaging to identify VRFs in root-filled teeth compared with direct visualization. The SHORT Case Of The Day! Tooth #3 ~ Extract? or Retrx? Patient came in pain on tooth #3 from a previous root canal by her dentist. She did not want an extraction and implant. Her dentist had challenges on this case so I agreed to help the patient keep the tooth. A LFOV CBCT was taken on tooth #3. The findings were as followed: *The MB and DB canals were missed. **Gutta percha was placed lateral to the MB and DB canal creating two perforations. ... ***The palatal canal was short. A retreatment was performed in one visit. I removed all the gutta percha. Located the original MB and DB canals. Cleaned and Shaped all the canals. Obturated the canals with gutta percha and bioceramic sealer. The perforations were sealed with MTA. Patient reported to be asymptomatic after treatment. A 6 month recall shows nice healing and the patient still asymptomatic. We all will have cases that will not turn out like we like. However, it's nice to give the tooth another chance before extraction. The SHORT Case Of The Day! Tooth #19 ~ Retrx with CBCT Patient came in with pain on the lower left side. There was a previous root canal on tooth 19 performed 10 years ago. There was a PAP on the distal root. A CBCT was performed which revealed pathology on both roots. Endodontic retreatment was performed in one visit using bioceramic sealer and gutta percha. Patient reported to be asymtomatic a few days later. She will be recalled in 6 months to evaluate healing. Tips...: There was a void due to cotton pellet left behind in the pulp space. This becomes a nitus for reinfection due to the leaky crown...which caused bacterial recontamination. In addition, look out for the "figure 8" shaped distal root. The anatomy was very interesting. The DB root wrapped around the Palatal root like a snake to a pole which represents the emblem of dentistry. Asclepius was one of the earlier gods associated with health. Asclepius was primarily known for his healing abilities, and his symbol was a staff entwined wi...th a snake. Believed to have taken the form of a serpent, the story goes that Asclepius healed the Greek people that were suffering from a Roman plague. They saw the shedding of its skin as a rejuvenation, a healing. Interestingly enough, the serpent has a healing representation in the Old Testament too! The SHORT Case Of The Day! Teeth #4 & #5 ~ CBCT Diagnosis & Trx Planning Patient came in with pain on the upper right. She said she had root canal treatment on both teeth a few years ago but the teeth never felt right. The 2D PAX shows pathology on tooth #5 but it appears to be sealed to the apex and tooth #4 filled short. I decided to perform a 3D CBCT in the area. It showed tooth #5 had a missed palatal canal with a post perforation on the buccal root in the apical 1/3. T...ooth #4 had a thickened PDL. Both teeth were percussion sensitive. Recommended a retreatment on both teeth with perforation repair with Bioceramics on tooth #5. Patient rescheduled for retreatment. Without the 3D CBCT I would not be able to see everything going on and would not know how to properly treat this case with a high success rate. *Key ~ This is why I don't recommend placing a post in the buccal root. The diagnosis was acute irreversible pulpitis and an incomplete or immature apex. What do you do? How you manage the patient? How do you manage the tooth? I recommend Apexogenesis on cases like these using nitrous oxide to relax the child and local anesthesia. Apexogensis was performed using ProRoot MTA Dentsply Sirona Endodontics on this case. I w...ould not use MTA on a front tooth due to staining from the bismuth oxide. I recommend Biodentine by Septodont North America or Bioceramic Putty by Brasseler USA or EdgeEndo or Avalon Biomed Inc. I published a step by step method of my technique of Apexogenesis. The SHORT Case Of The Day! Tooth #19 - Furcation Defect..Case Hogged Out... I hate that expression!...Just Extract? A Musical Experience Patient came in with pain associated with tooth #19. It had a previous root canal that was failing. The furcation bone was breaking down as well as around the roots. He did not want to extract the tooth and get an implant. His general dentist decided to send him to me for an evaluation and possible retreatment. Many dentist woul...d have just extracted the tooth based on how it looked on the xray. I understand. But let’s try to fix it first! The retreatment was performed in 2 visits with caoh. He returned asymptomatic and the case was completed with Kerr EWT sealer and warm vertical condensation. I know this case will drive the MIE disciples nuts...it’s too big....it’s too aggressive...blah blah blah....lol. Patient returned for a 2 year recall with nice healing at the apex and the furcation. Notice no puff....No lateral canal into the furcation...but amazing healing. No one knows how long this will last. The SHORT Case Of The Day! Tooth #28 - Bifurcation Anatomy Patient presented with pain associated with tooth #28. The diagnosis was a necrotic pulp with acute apical periodonitits. You can observe from the radiograph the pulp chamber is wide open then midroot is disappears or some say calcify. Well it's not really calcified. Calcification occurs coronally - apically. The main canal actually splits into smaller canals making it "tighter" which requires high magnfication ...and illumination. Typically on these cases they are bucal and lingual canals. Sometimes there could be a MB, DB, and L canal. This was a Vertucci Type III canal configuration. Endodontic treatment was performed with a conservative access in one visit using Edge Endo X7/04, WVC, with Kerr EWT sealer. Dr Rico D. The SHORT Case of The Day! Tooth #19 - Vertical Root Fracture or Strip Perforation? Extract and Implant? Patient came in with pain and swelling associated with a previous root canal on tooth #19. The dentist thought the root was cracked and understandably so due to the "J" shaped lesion on the mesial root. He recommended an extraction and implant. Patient wanted to try to save her tooth with a retreatment. (I would have taken a CBCT but did not have one at the time). Upon acc...ess with the surgical microscope, there was no vertical root fracture or crack however there was a strip perforation in the mesial root. The previous filling material was removed. I used CaOH as the interim medication. Patient returned in 2 weeks asymptomatic. Pro Root MTA was used to repair perforation in the mesial root with gutta percha in the distal. There was also a resorptive defect in the apical area of the mesial root. Notice the mesial canal was not filled to the radiographic apex. It happens to the best of us. The diagnosis was a necrotic pulp with internal resorption in the apical 1/3 that created a defect and large PAP. This case was performed in 2 visits using calcium hydroxide. Patient returned asymptomatic with no swelling. No antibiotics were prescribed. The case was completed using Bioceramic Gutta Percha Cone, Bioceramic Sealer, and Bioceramic Putty... by Brasseler USA. This created a solid 3D seal in the defect area. She will return in 6 months to evaluate radiographic healing. The SHORT Case Of The Day! Tooth #2 ~ Very Narrow Canals A Musical Experience Patient came in pain on tooth #2 due to deep decay to the pulp. Notice on the pre-op PAX the very narrow canals. When you have these cases it's important not to "force" files. I like to open up passively with small handfiles size 6,8, 10. Use an orifice opener then a glide path file close to length. Establish working length then use heat treated NiTi files in crown down fashion with .04 taper. ...Minimum size at the apex is .25 is the best so irrigants can reach according to studies by Chow. This case was obturated with bioceramic sealer vs warm vertical condensation because I could not get the heat within 5-7mm of the apex. #WorthSaving #RootCanals Dr. Rico D. The roots were very long almost 30mm, tricky anatomy, and patient had limited opening. Endodontic treatment was performed in one visit. The SHORT Case Of The Day! Tooth #14 ~ Retrx After Amalgam Apicoectomy with CBCT Patient came in pain on tooth 14. He had a previous root canal with a previous apico with amalgam retrofills and a large screw post in the palatal root. I perfomed a LFOV CBCT on this case. He did not want an extraction/implant. I decided to retreat this case. Upon access, I removed the post with ultrasonics cleaned/shaped the palatal canal to length, located the MB2 cleaned and shaped it, c...leaned and shaped the MB and DB up to the amalgam retrofill. I used Bioceramic Sealer and gutta percha to perform this case in one visit. The SHORT Case Of The Day! Teeth #3 and #4 ~ Deep Decay & Cool Anatomy Patient came in pain on teeth #3 and #4. Yes tooth #2 will need endo and so do a few others. But these were the ones hurting her at the time. She did not want to have the teeth extracted and implants so she decided to have root canals performed on both teeth. There was a lot of decay in both teeth evident on the PA and BW. In addition, the anatomy on both teeth were tricky...long and curved roots. Endodo...ntic treatment was performed with heat treated NiTi files and Bioceramic Sealer in one visit. Notice the anatomy at the apical 1/3 of tooth #4. A custom access cavity design is recommended for each tooth according to its static and dynamic occlusal relations. The SHORT Dental Tip of The Day! Did you know plain 3% mepivacaine (Carbocaine) solutions are just as effective as 2% xylocaine for an inferior alveolar nerve block? A cartridge of either mepivacaine or prilocaine will work the same as 2% lidocaine with epinephrine for pulpal anesthesia of at least 50-55 minutes. Clinically, this is an important finding because when medical conditions or drug therapies suggest... caution in administering epinephrine-containing solutions, plain solutions can be used as an alternative for the inferior alveolar nerve block. This is important for patients who are pregnant and who have had serious heart issues such as a recenct heart attack.
